This patient in her 50s came to us after having her initial augmentation about 15 years ago with a revision 5 years ago (with placement of 425 cc silicone implants). She was troubled by her asymmetry, her cleavage gap, and capsular contracture. She was also found to have stretching of the nipple-to-fold distance from implant weight over time (called pseudoptosis).She chose to undergo a revision procedure with implant removal, total capsulectomy, and placement of new silicone implants (560 cc silicone gel). She also had placement of surgical mesh to form an internal bra to offer implant support over time to prevent repeat stretching of the soft tissue envelope in addition to skin removal for correction of her pseudoptosis.
