This patient in her late 30s came to us after having her initial augmentation over 20 years ago. She then underwent a revision about 10 years ago with placement of subglandular silicone implants (339 cc). She developed capsular contracture over time with scarring and hardness of her breasts as well as significant asymmetry and desired correction.She chose to undergo a revision procedure with implant removal, total capsulectomy, conversion from a subglandular to a submuscular implant pocket, placement of new silicone implants (405 cc), as well as placement of acellular dermal matrix (biologic mesh) to support the implant, maintain the submuscular pocket, and prevent recurrent capsular contracture.
